Home Bound Policy
Students who, due to injury or for any other reason as certified by a licensed physician, and homebound for a period of two weeks or more shall receive home/hospital instructions.
Eligibility Criteria
Documentation that a student meets one of the following criteria:
Has an injury, communicable illness, or health condition that prevents her or him from attending school for a time that will interfere with the student's ability to master necessary skills and that is diagnosed and confirmed by a licensed physician; or
Has an injury or health problem/condition that requires her or him to be homebound or hospitalized for a period that has lasted or will last more than two weeks as diagnosed and confirmed by a licensed physician.
For students applying for homebound due to emotional illness, a treatment plan must accompany the homebound application. Typically the application must be reviewed and an updated physician statement required after 60 days.
Special Considerations for Eligibility:
A licensed physician must provide a written statement to the county school district that the student must remain at home or in the hospital for a period of two weeks or more. The written statement or application must include:
The specific reasons why the student must remain at home or in the hospital and
The criteria or conditions under which the student can return to school, and the expected date of such return.
Students placed on extended home instruction shall submit a physician's statement of need for continued home instruction when the term of home instruction reaches six months in length.
The county board may require that the parents obtain a second physician's opinion at the expense of the county board.
Schedule
Home Hospital instruction may be provided at any time. Instruction is to start as soon as possible following determination of student's eligibility.
The instructional schedule per week must be based upon the student's physical ability to attend/participate.
The time that instruction will be provided in the home is established by the teacher, the parents, and when appropriate, county school administrator.
Home instruction is provided at a time when a responsible adult is in the home.
The student's school shall provide the homebound teacher instructional materials; textbooks required, daily lesson plans, including homework assignments when requested.
